CASE MANAGEMENT

Case management is based on the premise that when one reaches their highest level of functioning, everyone wins - the individuals being served and their families - thus trickling down into the community at large. FOTFT can provide case management services in the areas of:

Re-Entry Case Management

Offers transitional support to men and women who are returning to the workforce
Develops partnerships with various resource networks
     -Meeting basic needs of each participant, Help facilitate a successful reentry
Conducts workshops on life skill areas
     -Goal Setting, Crisis Management, Communication Skills
Manages Case load of 25-30 men and women
     -Maintains current charts of each (including individual service plans, progress reports and encounter documents)
Holds individual meetings with each participants in various communities
     -Advocacy, linkage support to needed resources

Parenting Case Management

Pennsylvania Horticultural Society partnership

Most recently, Fruits of the Family Table partnered with Pennsylvania Horticultural Society to offer case management services for their Roots To Re-entry program.  Currently in its seventh year, this program provides intensive job training in landscaping and horticulture, as well as life skills support and job placement to inmates from the city’s prison system and those active on probation or parole. Program graduates are placed in high-demand jobs in the landscaping industry.
